Completed Interval Health History forms (Sports Triplicate) and a current Physical Exam (within the last calendar year) must be turned in to the Nurses Office prior to sports tryouts by email, fax, or regular mail. As a reminder, there are no physical exams offered at McQuaid. A new Interval Health History has to be completed every time a sport is done.
*The Nurses Office will accept forms for fall sports during the summer months. Please see the Interval Health History form below; this must be filled out, scanned and emailed, or faxed back to the nurse AT LEAST one week before tryouts begin for any particular sport.
